You are probably using RusMap, which causes this (see here)

This has been reported internally (#26418) already. Thanks for reporting regardless (next time, please include co-ordinates and a screenshot, as stated here).

I couldn't replicate this myself, have you tried with a ProMods-only profile?

Added to bugtracker (#26394, #26395, #26396 and #26397).
The last bug (Володимир-Волинський) is actually not a bug - these signs are based on how the signs look in real life.

The first co-ordinates were incorrect, these are the correct ones:
