Countries I lived:
Netherlands - born & bred, lived here until I was 28
UK - lived here since 2009

UK & Ireland:
of which: - Wales - visited about 10 times mixed work/private
of which: - Scotland - visited twice (Inverness for work and Lockerbie privately)
of which: - Northern Ireland - visited once, driving from ROI
Ireland - visited about 10 times (3 or 4 holidays the rest for work)

Western Europe:
Germany - visited so frequently I can't tell, more than 100 times
Belgium - visited so frequently I can't tell, more than 100 times
Luxembourg - visited here on many holidays and also work
France - visited here countless times as well
Monaco - visited once on holiday
Switzerland - visited here nearly 30 times for work and spent many youth holidays here with my parents, returned later for some trips as well
Austria - have spent at least 10 holidays here and several work trips
Liechtenstein - been here twice just to explore it

Southern Europe:
Spain - been here on holiday twice (Barcelona, Madrid) and a few times for work
Portugal - been here many times for work and holidayed in Lisboa, Sintra and Albufeira
Andorra - visited one driving back from Barcelona to the UK
Italy - only visited the north many times across the border from Austria & Slovenia and Milano & Venice for work
Greece - visited about 5 times for work and combined some holiday too (Thessaloniki, Athens and Crete)

Northern Europe:
Denmark - been here at least 20 times usually crossing through to Sweden but stayed around South Jylland, Falster and KBH.
Sweden - been here at least 30 times on holiday due to relatives and a dozen of times for work trips (used to work for a Swedish company)
Norway - been here three times on holiday and 7 times for work (mostly Oslo but also Bergen, never far north)
Finland - been here three times for work (Oulu, Helsinki twice)
Iceland - been here twice for flight connections to the US (could clearly see the volcanic black sand from Keflavik)

Baltics:
Estonia - visited once for work (Tallinn)
Latvia - visited once privately and once for work (Riga)

Eastern Europe:
Poland - visited about 7 times, private as well as work (Warsawa, Katowice, Krakow, Zakopane, Poznan, Lodz)
Czech Rep - visited about 7 times as well, mostly work and of course SCS! (Praha)
Hungary - visited about 3 times for work (Budapest and drove to Croatian border)
Slovenia - visited 4 times crossing through to Croatia (stayed once near Novo Mesto)
Croatia - visited 5 times, private/holiday (mostly Istria/Rijeka, Krk, Plitvice and Zagreb)
Bosnia Hercegovina - visited once (brief 2.5 hour crossing from Croatia to Bihac)

Middle East:
Turkey - visited 4 times for work (Istanbul only)
Israel - visited once for work (Tel Aviv, and Jerusalem & Dead Sea excursions)
Palestine - visited once near the Dead Sea
United Arab Emirates - visited Dubai for work

Africa:
Morocco - visited once on holiday
South Africa - visited once for work

Asia:
Japan - visited for work
Maldives - visited on honeymoon
Malaysia - visited on stop-over to Australia (Kuala Lumpur only)
Singapore - visited on stop-over to New Zealand

Oceania:
Australia - visited twice on holiday
New Zealand - visited for a month on holiday (wife is from here)

North America:
Canada - visited Windsor, ON and Niagara Falls, ON on different holidays. Toronto for work
USA, this requires a separate list:
2007 - Holiday trip to NYC [states: NY, NJ]
2008 - Holiday trip to Detroit & Canada followed by NYC [states: NY, MI]
2008 - Holiday trip to west coast for 4 weeks [states: CA, AZ, NV, UT]
2012 - Holiday trip to east coast for 2 weeks [states: NY, NJ, PA, DE, MD, DC, VA]
2012 - Work trip to Atlanta & Toronto [states: GA, TN, NC]
2014 - Holiday trip to west coast for 3 weeks [states: CA, AZ, NV UT]
2014 - Work trip to Florida & Las Vegas [states: FL, NV]
Mexico - crossed the border at Tecate whilst on holiday in 2008. Stayed about 1 hour.

Norway(my homecountry. Live in Bodø, but also been in Tromsø, Sortland, Harstad, Mo i Rana, Steinkjer, Trondheim, Lillehammer and Oslo.)
Sweden(visiting Sweden mostly every year. Live 2-3 hours away from the border. Been in Arjeplog, Arvidsjaur, Piteå, Skellefteå, Umeå, Sundsvall, Mora, Orsa)
Finland(was on a visit in Helsinki in 2011, while on a trip to Barcelona for a cruise in the Mediteranean Sea. Had to switch plane in Helsinki)
Germany(Cruise-ship from Oslo to Kiel in summer of 2014)
United Kingdom(school trip in 2010 to London)
Spain(In 2011 for a cruise-ship from Barcelona and around the western Mediterranean Sea)
France(Cruise-ship tour, Toulon and Villefranche-sur-Mer)
Italy(Cruise-ship tour. Civitavecchia and Naples
Greece(Only one time, at Crete, a popular tourist place called Agia Marina at the western parts of Crete. I have not been at the mainland of Greece before. I am going back to Crete in the summer of 2015.
Bulgaria(In 2007 at a popular tourist city called Sunny Beach). It is located at the coast of the Black Sea.

This is where the countries i have been to, but i am planning to travel elsewhere.

Btw this also explains my high amount of vehicles in the "what cars have you driven" topic because most of those were rental cars

Yeah, maybe that's true, who cab tell where I've been when I'm 33 years old!
I find ZelvaCZ list quite impressive too! My list when I was 14 would be about the same with the exception of Switzerland and Great Britain.
