Vatican City, no... possibly scenery only, viewable from a distance... but not to drive in. It's not possible to drive a truck in Vatican City in real life.

San Marino... yes, definitely! And it wouldn't be that hard to make fit... just position it a little further away from Rimini than it really is

I wouldn't be sure about the Vatican city, because to make it, there will be used some spaces which rather could be used for the Rome instead. I think Holy See is not worth to be added because of being so small - In the game, it is equal to nothing...

San Marino is different story, because it can fit, like Liechtenstein did once. @Superdavecan, I don't agree on your statement ("it wouldn't be that hard to make fit" - It can be quite hard to fit the country, you can try it by yourself

I think it would be very well, with my Grosseto city!!!!
I suggest also to add the 2 islands (Sardinia and Sicily), some more cities and real highways (autostrade) and secondary-streets (superstrade)...
