Size of San Marino (whole country) its half size of Milan or Naples. For example size of Liechtenstein is slightly bigger than Naples and almost same with Milan, but it's included in mod, so why no add San Marino?

It depends on what's around. Something else may occupy that area, for example the scenery of some highway.

If adding the correct interchange to San Marino, the map would need a rebuild in that area.

But for now there is no space between Technogym, Via Calcinaro, 2861, 47521 Cesena and Bacino del Conca. These are too close in SCS's Italy DLC to leave any space for the SS72 and A14 intersection to San Marino.

The intersection between SP423 andA14 to Pesaro is ingame, but not realistic to be used for San Marino.
Pesaro could possibly fit. But the current delivery spot might belong to Ancona?

In the future there may be ferries to the Balearic Islands, Corsica, Sardinia and Malta

So that means that those places are going to be done someday in the future.
