Knooppunt Kleinpolderplein is also nice and it will be in the Rotterdam Map as well...

PS: Check the height of the concrete barrier in the middle of the road at the prefab (for example pic1)

You could also select "asphalt_x_00" for the 5-lane road to make the transition look smoother...
